Health care involves a partnership between patients, families and health care providers. Each member of the partnership has certain rights and responsibilities. When you are well informed, participate in treatment decisions, and communicate openly with your health professionals, you help make your care as effective as possible. Privacy Notice

This Notice of Privacy Practices describes how we and the medical staff and personnel who provide you with care or services at this facility may use and disclose your Protected Health Information (“PHI”) to carry out treatment, payment or healthcare operations and for other purposes that are permitted or required by law.

Ownership Notification

During the course of your physician/patient relationship, your physician may at a future time refer you to one our outpatient facilities. In connection with any such referral, the physician hereby advises you that these physicians, listed in this notification, have an ownership interest. Please be advised that you have the right to obtain the health care items and services for which the physician refers you, at any location or from any ambulatory surgery center, hospital, provider or supplier of your choice, including Mississippi Valley Surgery Center or Mississippi Valley Endoscopy Center.

Rights and Protections Against Surprise Medical Bills

When you see a doctor or other health care provider, you may owe certain out-of-pocket costs, such as a copayment, coinsurance, and/or a deductible. You may have other costs or have to pay the entire bill if you see a provider or visit a healthcare facility that isn’t in your health plan’s network. “Out-of-network” describes providers and facilities that haven’t signed a contract with your health plan. Out-of-network providers may be permitted to bill you for the difference between what your plan agreed to pay and the full amount charged for a service. This is called “balance billing.” This amount is likely more than in-network costs for the same service and might not count toward your annual out-of-pocket limit. “Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care—like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider.
